This article explores the use of three-dimensional Non-Linear System (3D NLS) imaging in the diagnosis of Chronic Obstructive Pulmonary Disease (COPD), with a particular focus on emphysema and obliterating bronchiolitis. The study evaluated 91 patients with COPD symptoms and compared NLS findings with conventional chest radiography.
The authors report that 3D NLS imaging identified distinct patterns associated with different forms of emphysema, including centrilobular, paraseptal, and panlobular emphysema, as well as signs of obliterating bronchiolitis such as air trapping, bronchial wall abnormalities, and bronchiectasis. In many patients, emphysema and bronchiolitis coexisted, creating characteristic imaging patterns.
The study found that respiratory symptoms, particularly shortness of breath, correlated more strongly with obliterating bronchiolitis than with emphysema. Additionally, conventional chest X-rays frequently failed to detect mild-to-moderate emphysema or produced inaccurate results, while 3D NLS imaging was described as more sensitive for identifying early structural changes.
The authors conclude that COPD involves at least two parallel pathological processes—emphysema and obliterating bronchiolitis—and suggest that 3D NLS imaging may offer a more detailed assessment of disease patterns and progression than traditional radiographic methods.
